Tom S. Maddox, Jr, M.D. received his B.A. from the Murray State University and his Medical Degree from the University of Kentucky. After internship training at Tulane in Charity Hospital of New Orleans, Louisiana, and residency training at the University of Kentucky, he was certified by the American Board of Ophthalmology in 1975.
   

David E. Jones, M.D. attended the University of Kentucky. He received his Medical Degree from the University of Louisville School of Medicine. He completed his internship, residency training, and research fellowship in ophthalmology at Ohio State University. He was certified by the American Board of Ophthalmology in 1983.
   

C. Mark Millsap, M.D. received his B.S. from East Central University, Oklahoma, and his Master's Degree and Medical Degree from the University of Oklahoma. After internship training at the University of Oklahoma, he completed residency training at Texas A & M and a Vitreoretinal Fellowship in New Orleans at Louisiana State University. He was certified by the American Board of Ophthalmology in 1994.
   

Thomas G. Furgason, M.D. received his B.A. from Williams College and his Medical Degree from the University of Kentucky. After internship training at Christ Hospital in Cincinnati, he completed residency training at the University of Wisconsin in 1998.  He was certified by the American Board of Ophthalmology in 2000.
   

Troy L Haleman, M.D. received his B.S. from Kentucky Wesleyan College and his medical degree from the University of Louisville.  he completed his internship at the University of Louisville and his residency training in ophthalmology at the University of Alabama at Birmingham.  He was certified by the American Board of Ophthalmology in 2002.
